If you are vaccinated with the COVID-19 vaccine, you will be able to download a vaccine certificate with QR code. Your vaccine certificate is an electronic record of your COVID-19 vaccination, including information about each dose, which can serve as a personal health record and/or proof of vaccination.

Effective March 1, 2022 Ontario no longer requires businesses and organizations to check for proof of vaccination. However, individual businesses and organizations may choose to require proof of vaccination upon entry.

Individuals also may be required to show proof of vaccination when travelling within and outside of Canada. Check applicable requirements at destination.

Get your vaccine certificate

Step 1: get your vaccine certificate with QR code

There are several ways to access your vaccine certificate.

Online

You can download your vaccine certificate from Ontario's vaccination portal. To log in securely, you need:

  • a green photo health (OHIP) card (you will need numbers from both the front and back of the card, expired cards will be accepted)
  • your date of birth
  • your postal code (the one associated with your health card)

By phone

If you do not have a computer, a green photo health card, or need help, call the Provincial Vaccine Contact Centre at 1-833-943-3900 to request a copy of your vaccine certificate by email or mail.

You can also:

  • visit your local library to use a public computer and internet resources
  • ask a trusted friend or organization to help print a copy
  • access a Service Ontario location

If you have a red and white health card

Call the Provincial Vaccine Contact Centre at 1-833-943-3900 and they will email a secure URL through which individuals can access their vaccine certificate.

If you don't have any health card

You can contact your public health unit to have your identity verified and get a  ID (a unique number assigned to you by your public health unit in place of an Ontario health card number to obtain a copy of your vaccine certificate). You can then call the Provincial Vaccine Contact Centre at 1-833-943-3900 and the contact center agent can email you a secure URL to access your vaccine receipt.

Step 2: save or print

After you've logged in, you can:

  • save a digital copy of your receipt to your device
  • save a screenshot to your device
  • print a paper copy

Proof of Vaccination

You are considered fully vaccinated in Ontario if you have received:

  • the full series of a COVID-19 vaccine authorized by Health Canada, or any combination of such vaccines (two doses of Moderna, Pfizer-BioNTech, AstraZeneca, including COVISHIELD) in any combination or one dose of Janssen (Johnson & Johnson)
  • one or two doses of a COVID-19 vaccine not authorized by Health followed by one dose of a COVID-19 mRNA vaccine authorized by Health Canada
  • three doses of a COVID-19 vaccine not authorized by Health Canada
  • your final dose of the COVID-19 vaccine at least 14 days before providing the proof of being fully vaccinated.

If you received one or more of your doses outside of Ontario

If you received one or more doses of the COVID-19 vaccine out of Ontario, contact your local public health unit to verify and register your information in order to book any additional doses if needed and to download a vaccine certificate.

Once recorded, you will be able to book a second or booster dose if needed and obtain a vaccine certificate through the provincial portal at COVID-19 or by calling the Provincial Vaccine Contact Centre (PVCC) at 1-833-943-3900.

If you have applied to have your out of province vaccination(s) registered with your public health unit and are waiting to have them uploaded, you can call the Provincial Vaccine Contact Centre at 1-833-943-3900 to book your next vaccine appointment if you fit the following criteria:

  • you have an Ontario health card or COVID ID (a unique number assigned to you by your public health unit in place of an Ontario health card number)
  • your public health unit uses the provincial booking system
  • you are able to bring your out of province vaccination receipts to your vaccination appointment for validation

Medical and  vaccine clinical trial exemptions

If you are unable to receive the COVID-19 vaccine due for medical reason or participation in a vaccine clinical trial, you may wish to obtain a vaccine certificate with this information.

To obtain a vaccine certificate for a medical exemption:

  • ask an Ontario physician or registered nurse in the extended class to submit your exemption request directly to the public health unit where the issuer’s practice is located
  • once your exemption is accepted and entered into COVaxON by the public health unit, you’ll be able to access your vaccine certificate with QR code through the COVID-19 vaccination portal

Processes may vary in different public health units. Contact your primary health provider or nurse practitioner to learn more.

To obtain a vaccine certificate with a vaccine trial exemption:

  • your vaccine trial Principal Investigator or Study Director can submit an exemption, with your consent, directly to the Provincial Vaccine Contact Centre
  • once your exemption is reviewed and entered into COVaxON by the public health unit, you can access your vaccine certificate with QR code through the COVID-19 vaccination portal.
