Check out our new secure online system for reporting your child’s immunizations to Chatham-Kent Public Health called ICON (Immunization Connect Ontario). You can also view your child’s current, overdue and upcoming immunizations. It’s like a yellow card, only better.
Why should I use this site?
- It’s an online alternative to paper immunization records.
- It allows you to securely keep track of your child’s immunization record.
- It allows you to report your child’s immunization records to us at the health unit, as required by the Immunization of School Pupils Act, 1990 (ISPA) and the Child Care and Early Years Act, 2014 (CCEYA).
How do I use this site?
It’s easy! Every time your child gets a vaccine, report it online at www.ckphu.com/vaccination . The vaccine is automatically submitted to the health unit and you’ll have a complete record of your child’s immunizations.
To view your child’s record, you will need their Ontario Immunization ID to create a secure PIN. To obtain your child’s Ontario Immunization ID or for more information about ICON and immunizations, please contact Chatham-Kent Public Health at 519.355.1071 ext 5900.
