Vaccine Records

You must update your immunization record with Public Health.

If you aren’t sure if you have all your vaccines, call your healthcare provider or contact us. We can check your vaccine record and help you get any missing vaccines.


Your Record

It’s important to have a record of your vaccinations. You may need it to travel, work, or volunteer. Kids need vaccine records to sign up for school, daycare, and other programs. 

In Ontario, most vaccine records are written on a yellow card by your doctor. Doctors, schools, and daycare centers don’t send these records to public health. In Chatham-Kent, families need to update their vaccine records with CK Public Health.

Ontario Vaccine Schedule

In Ontario, Public Health and healthcare providers follow the Ontario Immunization Schedule. Some vaccines are needed for school and childcare. Others are optional, but they provide extra protection. Call us to learn more information or to talk about Immunization Through the Lifespan.

Do Vaccines Work?

Yes! Vaccines work so well that most of the diseases they protect against are now very rare. Did you know that Canada was one of the first countries to get rid of polio by vaccinating people? When most people get vaccinated, diseases don't spread much. Without vaccines, there would be more outbreaks of diseases that we are now protected from
