Vaccine Records

You must update your child's immunization record with CK Public Health. 

If you aren't sure if your child has all of their vaccines, call your healthcare provider or contact us. We can check your child's vaccine record and help you get them any missing vaccines.

Update Your Record

You can update your immunization record with CK Public Health in a few ways:

  1. Visit ICON using your Ontario health card and update the online record.
  2. Contact us and speak with a public health nurse who can update your record over the phone.
  3. Fax a copy of your vaccine record to CK Public Health at 519-436-3214
  4. Visit us with your vaccine record at 435 Grand Ave West, Chatham


Vaccine Requirements for School & Childcare

As a Public Health Unit, we ensure that children in Chatham-Kent have their vaccines up to date or have a valid exemption if they go to school or daycare. These requirements are set by Ontario's Immunization of School Pupils Act (ISPA) and the Child Care and Early Years Act.  

Starting School or Daycare

Before your child starts school or daycare, please report your child’s vaccinations to us. You can do this online using ICON, or fill out the school or daycare form and send it to CK Public Health with a copy of your child's immunization record. If you have any questions, contact us for help!  



Grade 7 Vaccine Program

In Ontario, Public Health Nurses visit schools to give three important vaccines to Grade 7 students. These vaccines work best when kids are younger and help protect them from serious illnesses.  

  1. Meningococcal: Protects against bacteria that can cause dangerous infections in the brain, spine, and blood. The vaccine is about 80%-85% effective in preventing infection from 4 types of meningococcal bacteria.  
  2. Hepatitis B: Protects against a virus that can cause liver damage and liver cancer. After all doses of the vaccine, it is 95% to 100% effective in preventing chronic hepatitis B. 
  3. HPV: Protects against a virus that can lead to different types of cancer, including cervical and anal cancers. It is almost 100% effective at preventing HPV infections that can cause cancer! Check out this video to learn more!

    Consent Package

    In September, Grade 7 students get a consent package to take home and complete. Vaccines will only be given to students who return the completed form.  



    Vaccine Exemptions

    CK Public Health recommends that all children and adults get routine vaccines unless there is medical reason not to. If you decide not to vaccinate your child, it is important to understand the risks.  

    In Ontario, children who are not up to date with required vaccines must have a completed exemption form to stay in school or daycare. Every family is different, so it's a good idea to talk to your child's healthcare provider about what exemption option is best for your situation. You can always change your mind and join the Ontario Vaccination Program later, even if you had an exemption before.  


    Medical Exemptions

    If your child can't get some or all of the vaccines for medical reasons, you and their healthcare provider need to fill out a medical exemption form. After the form is filled out, you must bring it to CK Public Health in person at 435 Grand Ave, Chatham ON.
