Naloxone Reporting Form

The Naloxone Reporting Form is used to report program-related information. This reporting is a requirement of the Naloxone Community Partners Program, and the information collected is submitted to the Ontario Ministry of Health.

Please note the reporting deadlines for each quarter listed below and ensure your completed form is submitted on or before the applicable due date.

Reporting Periods

Q1: April 1 - June 30 | Due July 10

Q2: July 30 - September 30 | Due October 10

Q3: October 1 - December 31 | Due January 10

Q4: January 1 - March 31 | Due April 10
