Upcoming Immunization Clinic Dates for Students



In order to protect children against vaccine preventable disease, the Chatham-Kent Public Health Unit is mandated by the Ontario Immunization of School Pupils Act to collect vaccine information from each student enrolled in school. If the Health Unit does not have the required immunization information or a valid exemption, then you have been issued an Order for Suspension from Attendance at School as directed by our Medical Officer of Health, Dr. David Colby.

***     If we still have not received the necessary information before March 27, 2018    ***
your child will not be permitted to attend school.

Please choose ONE (1) of the following to avoid suspension from school:

1) See your family doctor to update your record and/or receive the necessary shots. Take the Order for Suspension form with you. Once you receive the vaccine(s) or the updated record, notify Chatham-Kent Public Health Unit by any of the following:

2) If you do not have a family physician or nurse practitioner and need to receive immunizations, call the phone number above to book an appointment to receive the necessary shot. Bring the suspension order with you. You must also bring with you a record of ALL previous shots received so that the nurse can administer the appropriate shot(s).
