Are your records up to date? School suspensions.

In order to protect children in school against vaccine preventable disease outbreaks, the Chatham-Kent Public Health Unit is mandated by the Ontario Immunization of School Pupils Act to collect vaccine information from each student enrolled in school. If the Health Unit does not have the required immunization information or a valid exemption, then you have been issued an Order for Suspension from Attendance at School as directed by our Acting Medical Officer of Health, Dr Colby. If we still have not received the necessary information before June 1, 2011 your child will not be permitted to attend school.

Please choose ONE of the following steps to clear the suspension before the order takes effect:

1)     See your family doctor to update your record or receive the necessary shots.  Take the Order for Suspension form with you.  Once you receive the vaccine(s) or the updated record, notify Chatham-Kent Public Health Unit by any of the following:

2)   Bring a copy of the immunization record to Public Health Unit for review to 177 King St E or:

 3)     Call the Chatham-Kent Public Health Unit at (519)-355-1071 – Ext. 5900 for an appointment to receive the necessary shot.  Bring the suspension order with you. You must also bring with you a record of ALL previous shots received so that the nurse can administer the appropriate shot.
